  • What type of patients can benefit most from Aspire Health's services?

    Aspire Health treats patients with complex conditions or suffering from serious illnesses such as cancer, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), kidney failure, liver failure, advanced dementia, and Amyotrophic Lateral Sclerosis (ALS). Aspire’s services are also appropriate for patients with advanced stages of these diseases (e.g., Stage IV cancer) and/or patients who have earlier stages of these diseases but also have frequent visits to the hospital.

    Aspire Health provides specialized medical care for patients with complex conditions or facing a serious illness. Aspire’s services focus on helping patients manage their conditions and providing patients with relief from the symptoms, pain, and stress of a serious illness. The goal is to improve quality of life for both patients and their families. Working together with a patient’s other doctors, Aspire’s clinicians manage symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping and depression. Aspire’s services help patients gain the strength they need to carry on with daily life and tolerate medical treatment, navigate the healthcare system, and make difficult and complex treatment choices. Aspire’s providers work to offer patients and their families the emotional and spiritual support that is critical in these challenging situations.

  • Who provides Aspire Health’s services?

    Aspire Health’s services are provided by a team of physicians, nurse practitioners, nurses, and social workers who specialize in caring for patients facing a serious illness. Aspire’s clinicians work together with patients, their families and their physicians to ensure that Aspire’s services are tailored to the specific needs of each patient and family.

  • Where can I receive Aspire Health’s services?

    Patients with specific health insurance plans are eligible to have Aspire Health’s clinicians come to their home for routine and urgent visits or have virtual visits. They also receive 24/7 support, including 24/7 virtual access to doctors or advance practice providers who can assess a serious medical situation and schedule as-needed visits in the event of an urgent situation or crisis.

  • Will Aspire Health visit me in my home?

    Yes, Aspire Health does offer home visits from a nurse practitioner or physician assistant. Aspire is currently only able to offer home visits to patients enrolled in specified health plans and in certain markets.   • Does Aspire Health provide 24/7 support?

    Yes, Aspire Health provides 24/7 support – including home and virtual visits from an advanced practice provider (nurse practitioner or physician assistant) in the event of an urgent situation or crisis – to patients with certain health insurance plans.

  • How does Aspire Health work with a patient’s primary care and specialist physicians?

    Aspire Health’s clinicians work in partnership with a patient’s primary care and specialist physicians. Aspire’s clinicians communicate with a patient’s other physicians on a regular basis and share any recommendations for changes to a patient’s care plan directly with that patient’s physicians. In addition, Aspire provides each of its patients with a one-page reference sheet that contains critical information such as a patient’s diagnosis, medications and treatment goals that can be given to any medical professional at any time.

  • How does Aspire Health work with patient’s home health providers?

    Aspire Health partners with home health companies to provide support to patients who are eligible for home health services. Aspire’s clinicians communicate with a patient’s home health agency on a regular basis about any recommended changes to a patient’s care plan. In addition, Aspire provides each of its patients with a one-page reference sheet that contains critical information such as a patient’s diagnosis, medications and treatment goals that can be given to any medical professional at any time.

  • Do I need a referral to Aspire Health?

    Yes, patients typically need a referral from their PCP, specialist physician or health plan in order to see Aspire Health’s care team. If you are having difficulty obtaining a referral or are not sure how to obtain a referral, please call Aspire at (844) 232-0500 (Toll Free).
